Phase 5 Ticket Request Form
Every member of the band (including guard members) may request a Phase 5 ticket for each game they are NOT assigned to play.
A drawing will be held one week before each game and the list of those selected to receive tickets will be distributed via email.
Those selected to receive a Phase 5 ticket must come by the band office during regular business hours to pick-up their ticket.
The office will not be open on Saturdays or Sundays so tickets for weekend games must be picked-up by 5p Friday.
If your name appears on the list to receive a ticket and you cannot use the ticket, notify EL and she will let the next person on the list know to come by the office and get a ticket.
All normal entrance policies printed on the ticket apply to these Phase 5 tickets.
These Phase 5 tickets must be used by band member - they cannot be given to anyone else.
